Township Clerk
Passport Application Information
Applications are accepted Monday through Friday, 8:00am - 3:30pm.
All Applications must have the following:
- Original birth certificate, naturalization certificate or passport. This information will be mailed with your application to the
passport center and be returned to you.
- Current and valid drivers license or photo identification card. Must also have a copy to be sent with the application.
- Two (2) identical passport pictures. Photos available
on site.
- One (1) check/money order made out to the U.S. Department of State.

You will receive your passport in approximately 4 - 6 weeks.
